The Advanced Practice Nurse Practitioner is a clinical leader responsible for providing guidance and support to the interdisciplinary care team serving our renal patient population in Puerto Rico. You will provide direct guidance to the Interwell Health interdisciplinary team regarding care plans and integrated care coordination. You will communicate with Primary Care Physicians and Nephrologists in both English and Spanish in the effort to provide exceptional care and improved health outcomes.

The work you will do:

  • Contribute to the development and continuous improvement of the Interwell Health clinical program and offerings, following the most up to date guidelines for the care of persons with CKD and ESKD.
  • Review escalated care plans created by the registered nurse and interdisciplinary team, and provide recommendations appropriate within the scope of service, with a focus on evidence-based and holistic care, including social determinants of health and transition management.
  • Partner closely with providers and dialysis clinics to identify and mitigate hospitalization and mortality risk for members, remove barriers to care and ensure a successful transition from chronic kidney disease to their renal replacement therapy of choice.
  • Lead clinical care rounds to address changes in patient status and ensure appropriate updates to the care plan.
  • Review available labs and other biometrics and discuss interventions with RNs and providers according to Interwell Health protocols.


The skills and qualifications you need:

  • Master’s degree or higher in the Science of Nursing, and Advanced Practice Nurse or Nurse Practitioner unrestricted license in Puerto Rico required.
  • 3+ years of experience in the delivery of care to patients with CKD and ESRD OR other chronic disease management, as an advanced practice provider.
  • Experience with home dialysis therapies including peritoneal dialysis and home hemodialysis.
  • Experience providing care through telehealth or other technology enabled platforms preferred.
  • Demonstrated ability to lead interdisciplinary teams in the development of individualized patient centric care plans.
  • The ability to critically assess and help RNs to prioritize a panel of patients through multiple forms of communication (i.e., phone, secure online chat).
  • Passionate about developing innovative clinical programs, improving patient outcomes, building a culture of safety and world-class care, and advancing Interwell Health’s mission
  • Must be bilingual - Spanish - written and verbal professional proficiency required.
  • Must reside locally and be able to travel to practices as needed throughout Puerto Rico.
