Experience & Requirements:

  • Graduate of an accredited School of Nursing.
  • Current valid state license Licensed Practical Nurse in the State of Alabama.
  • Possesses an active CPR certification.
  • Remains knowledgeable about specific state laws and regulations governing practice.
  • Satisfactory completion of initial and annual clinical competencies to demonstrate aptitude as assigned by role.
  • Solid clinical judgment and problem solving ability .
  • Experience using Excel, Word, and Outlook
  • Effective time management skills
  • Bi-Lingual in Spanish and English
  • Subject to initial and ongoing security clearance requirement
A background check, security clearance and drug screening are required for this role.

The hourly rate is between $27.20 to $35.50 per hour. Final rate is based on years of experience and shift differentials.
  • Addresses barriers to care management and self-care.
  • Coaches and guides the patient to meet both personal and clinical goals.
  • Facilitates and enhance the connection between patient and family, providing supportive presence to both the patient and the care environment.
  • Documents important information regarding patient’s medical and/or mental well-being, needs, security concerns, and any other pertinent information.
  • Collaborates with appropriate staff to communicate special needs or concerns of the patient.
  • Participates in care team meetings.
  • Mitigates complaints and ensures proper documentation by working closely with care team
  • Tracks all patient contacts, follow ups, barriers to care and outcomes
  • Follows standard precautions using personal protective equipment as required.
  • Will interact with State Agencies to include the ADOC, ADH, Social Services, etc to address any issues or concerns they may have during incarceration or at discharge.
  • Other duties as assigned by management.
