TITLE: HomeCare Field Service Supervisor (RN)
REPORTS TO: Director of Nursing
RESPONSIBILITIES:
- Provide field supervision, monitoring, and evaluation of the impact and efficacy of home care service provision on Agency assigned caseload.
- Conduct nursing assessment for initial cases, when required, to determine eligibility for service.
- Conduct nursing assessment visits according to an Agency prescribed visit frequency in order to evaluate client status. Submit such visits in an Agency approved format on a timely basis.
- Conduct nursing supervisory visits based on an Agency determined visit frequency in order to supervise home attendant performance. Submit such visits in an Agency approved format on a timely basis.
- Provide on the job training as needed to Agency employees.
- Evaluate the performance of PCAs and HHAs assigned to clients within caseload.
- Conduct quarterly competency reviews of home care employees.
- Notify Service Quality Supervisor of any environmental, social or service problems noted on regular visits.
- Notify Service Quality Supervisor OF Case Coordinator of recommendations for termination or discipline of PCAs and HHAs assigned to caseload based on information gathered in supervisory visits, and from conducting quarterly competency reviews or as a result of conducting performances evaluations.
- Notify Intake Coordinator immediately of any client complaints received.
- Conduct home visits for non-serious (Level IV) complaints when warranted. Recommend disciplinary action for home care worker if warranted.
- Serve as back up to Service Quality Supervisor to conduct required home visits for level I, II and III complaints.
- Conduct cluster care assessments for clients entering into cluster service.
- Complete cluster care task analysis sheets for clients eligible for cluster care.
- Evaluate assigned clients to determine the effectiveness of service efficiencies on an as needed basis.
- Evaluate assigned clients to assist in the determination of eligibility and service authorization as assigned by the Agency.
- Advise existing clients regarding Agency policies with regard to advance directives. Assure client completion of appropriate forms.
