PRN Visits start at $75/visit and will increase depending on the type of visits made.

Mileage is reimbursed at .65/mile traveled (not including first and last trip of the day).

St Paul Hospice Care is looking for an experienced Hospice RN to join our team of hospice professionals and provide quality care to our patients and their families .Must be a registered nurse in the State of Arizona with a current license and be a licensed driver with an automobile that is insured in accordance with state.

Knowledge of governmental regulations and CHAP standards are essential.

Ability to deal tactfully with patients, staff, and the community.

Work directly with the DCS, the Administrator and Medical Director in a mutually respectful environment.

Responsibilities:

  • Responsible for supervising hospice aides
  • Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs
  • Provides a complete physical assessment and history of current and previous illness(es)
  • Provides professional nursing care by utilizing all elements of nursing process
  • Assesses and evaluates patient’s status
  • Creates the plan of care and makes necessary revisions as patient status and needs change
  • Includes the patient and the family in the planning process
  • Uses health assessment data to determine nursing diagnosis
  • Provides health care instructions to the patient as appropriate per assessment and plan
  • Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care
  • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload
  • Completes, maintains and submits accurate and relevant clinical notes
  • Communicates with the physician regarding the patient’s needs
  • Teaches the patient and family/caregiver self-care techniques as appropriate
  • Works in concert with the interdisciplinary group
  • Ensures that arrangements for equipment and other necessary items and services are available
  • Supervises ancillary personnel and delegates responsibilities when required
  • Fulfills the obligation of requested and/or accepted case assignments
  • Participates in on-call duties as defined by the on-call policy

Qualifications:

  • Graduate of accredited school of nursing
  • Current Arizona RN licensure in good standing
  • Minimum two years of recent experience in medical, surgical or critical care as a professional nurse
  • Strong clinical assessment skills
  • Excellent communication and interpersonal skills
  • Solution driven, creative and resourceful problem solving skills
  • Ability to manage time well and perform assigned duties with attention detail, accuracy and follow-through
  • Proficient computer skills
  • Previous hospice or oncology experience
  • Knowledge of the death/dying and bereavement process

Pay: $75.00 per hour
