MHC Healthcare is seeking a bilingual Nurse Practitioner with at least 1 year of recent Primary Care or Family Practice experience to join our Integrated Care Team in at the MHC West Side Health Center, located in Tucson, AZ. MHC Healthcare is a Federally Qualified Community Health Center (FQHC), with 17 sites in Tucson and Pima County. The Family Nurse Practitioner promotes and maintains patients health by providing medical services to the extent to which is licensed and/or certified. The Nurse Practitioner uses independent judgment in carrying out daily job responsibilities. The position will be supervised by the Medical Director. The ideal candidate will have the following education and experience:
- Master’s degree (MSN) from an accredited school of nursing, combined with at least 1 year of recent Primary Care or Family Practice experience.
- Candidate without the MSN, but whose Nurse Practitioner certification is current and was grandfathered-in prior to the Master’s degree becoming a requirement for the credential will be considered if they also have required hands-on experience.
- Bilingual (English/Spanish)
Duties and Responsibilities of the Nurse Practitioner – Family Practice to include, but not limited to:
- Under the general supervision of the Medical Director, plans, directs, and evaluates patients in primary care setting.
- Identifies short-term and long-range patient care issues that must be addressed; providing information and commentary pertinent to deliberations; recommends options and course of action.
- Refers patients to physicians, physicians of specialty, as needed, with written referral indication proper documentation needed to continue care for patients.
- Collaborates with other physicians in the care of patients as needed.
- Provides continuity of care by developing and implementing patient management plans.
- Diagnose and treat common acute health problems.
- Diagnose, treat, and monitor chronic health problems.
- Provide health maintenance care such as physical examinations for all ages, well baby checks and gynecological care.
- Collaborate with physician as needed on complex, unusual, and high-risk cases.
- Order and interpret diagnostic tests and prescription of proper medications. Follow-up with patients through telephone and mail correspondence. Document patient contracts in patient chart.
- Promote wellness and self-care of patients and their families by providing health education including counseling on exercise, nutrition, well baby care, family planning, and diabetic issues.
- Perform out-patient procedures such as suture removal and other procedures within scope of practice.
- Attend continuing education conferences as required to maintain licensure.
- Promote community awareness on health care issues.
- Promote and participate in off-site activities such as Care-Fairs, School Base Programs, and Marana Treatment Center as requested by Medical Director.
- Ensures proper documentation on patient charting, patient encounter forms. Insurance forms and any other additional information pertaining to patient care.
- Participates in other assigned duties as designated by the Medical Director and/or Chief Executive Officer.
