Overview:
Marin Community Clinics, founded in 1972, is today, a multi-clinic network with a wide array of integrated primary care, dental, behavioral, specialty and referral services. As a Federally Qualified Health Center (FQHC), we provide vital health services to almost 40,000 individuals annually in Marin County. The Triage RN position provides nurse triage services and health advice for MCC patients over the phone at our West San Rafael Clinic. The RN assesses and documents patient questions or concerns and provides nursing advice using approved evidence-based triage protocols. The RN schedules follow-up appointments or refers to emergency services as needed. Triage RNs gain experience in triage of a variety of acute and chronic conditions for our diverse patient population. Furthermore, they are able to improve patient care through health navigation and chronic illness self-management coaching; these are skills that are unique to our primary care setting and are a great opportunity to make a difference in the lives of our patients, many of whom need assistance with navigating health systems and their own conditions.
This RN Triage position’s primary focus is triaging patients via telephone. Additionally, they provide on-site support for the care team. This involves heavy use of EHR and instant messaging for coordination of patient care. This position also has the possibility of working weekends and evenings.
- Bilingual English/Spanish skills are required.
- 3+ years of direct patient care experience required; community health and/or triage experience preferred.
Responsibilities:
- Provides excellent customer service in all phone and in-person interactions.
- Provides culturally sensitive, patient-centered care to all MCC patients.
- Provides nursing care within scope of license and clinic policy and procedures.
- Provides initial and follow up education for disease processes, such as, diabetes, hypertension, medication education, etc.
- Uses protocol, clinical judgment, and experience to designate an appropriate disposition for patients calling in for medical advice.
- Documents subjective and objective information as well as a patient plan for all triages.
- Assist providers and perform procedures accurately and thoroughly per protocol and as demonstrated on the skills assessment. Such as: urinary catheterizations, phlebotomy, wound care, PT/INR, and other POC tests as ordered by provider.
- Administers injectables including insulin, antibiotics, Toradol, pain medication, Kenalog, and immunizations following standard procedures and as demonstrated on the skills assessment.
- Administer and monitor oxygen and nebulizers per protocol and as demonstrated successfully during the skills assessment.
- Conducts assessments and triage of pediatric, adult, and gynecologic medical complaints by phone and in-person using evidence-based protocol.
- Elicits history of present illness and pertinent past medical history to formulate nursing assessment and plan for all triages.
- Collaborates with patients’ primary care team as needed to coordinate follow-up care.
- Reviews provider result folders for urgent lab and diagnostic results and alerts the provider as per protocol.
- Follows through in a timely manner on patient requests that come through in electronic in baskets and via patient portal.
- Reviews and follows up Emergency Department and Hospital discharge summaries to facilitate coordination of transition to home.
- Responds to patient requests for lab results, medical questions and questions and concerns regarding disease processes.
- Sends medication refills per clinic protocol.
- Orders medications such as emergency contraception per standardized procedure.
- Assists in the development, maintenance, revision and evaluation of clinical workflows.
- Reacts in a positive manner to patient concerns; remains calm and displays empathy.
- Works cooperatively with other staff and departments to problem resolve conflicts and improve workflow.
- Responds to and manages emergencies as per protocol.
- Maintains crash cart and orders supplies as needed.
- Maintains patient confidentiality.
- Participates in required meetings.
Qualifications:
Education and Experience:
- Associate or BA/BS degree required.
- Current California RN License in good standing.
- Current CPR/BLS certification.
- 3+ years of direct patient care experience required; community health and/or triage experience preferred.
- Experience with EPIC Electronic Health Records preferred.

Physical Requirements and Working Conditions:
- Prolonged periods of typing and sitting at a computer.
- Must be able to lift up to 15 pounds at a time.
Covid-19 Vaccination and Boosters Requirements: All employees are required to be fully vaccinated for COVID-19, including "health-care" required boosters as a condition of employment, subject to limited exemptions. New employees are required to provide proof of being fully vaccinated for COVID-19 and boosted before the first day of employment. If you completed your primary series, received an original booster and it has been more than 2 months from that booster dose - it is recommended, but not required that you receive the Bivalent booster.
