The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco. We are currently looking for qualified candidates for our PrEP Navigator role. Please note: this role requires an applicant to be bilingual (English/Spanish). This position reports to the HIV Prevention and Wellness Manager and is a Regular, Full Time, Non-Exempt role with a starting hourly rate range of $22.44 to $25.26 with full benefits.
Primary Objective:
The PrEP Patient Navigator will assist PCHP Grant Team with program implementation. He/she will help patient’s secure PrEP, help with medication adherence and retention on PrEP, facilitate testing for HIV, assist as part of a team to deliver HIV test results to patients, conduct patient follow-up, and participate in PrEP promotional campaigns, outreach, data entry and other duties, as needed. The PrEP patient navigator will take the lead with those testing negative to ensure the patient is educated about safe behaviors and PrEP, receives the necessary lab work to start PrEP, and is assisted with the costs of medication so as not be a barrier to adherence and patient care.
Essential Functions/Responsibilities:
PrEP Clinic Outreach
- Collaborates with project staff in advertising and media promotion of the HIV testing program;
- Responsible for facilitating HIV antibodies testing on-site and through outreach into the community, including other community settings where the target population socializes and/or congregate as needed;
- Conduct street and community outreach to target populations at various venues such as support groups and agency presentations, for which morning, night, and weekend availability is required.
- Assist in coordinating online outreach efforts, including social media management, content creation, blog posts, email blasts, etc.
- Attend events and street fairs to provide PrEP Outreach
PrEP Navigation Services
- Provides confidential assessment to determine the HIV prevention needs of the clients whether test results are positive or negative; this includes the registration, and planning of prevention services;
- Develops PrEP/PEP resources available in the area for the clients in need of the services;
- Develops a network with PrEP/PEP providers;
- Provides access to prevention services (PrEP/PEP), ensuring access, follow-up, and adherence to PrEP/PEP, including supportive services that can enhance risk reduction, behavior change, adoption, and maintenance;
- Provides and ensures access and active referrals to the screening of STIs;
HIV Linkage to Care
- Provides HIV test results to individuals who have tested for HIV;
- Offers Partner Services to individuals who test positive for HIV;
- Performs follow-up risk assessments with individuals who test negative for HIV to reinforce the individual’s realistic perception of risk;
- Collaborates with Pharmacist, Provider and TLC or RRC for linkage, and re-linkage to prevention services, STIs screening, and follow up;
- Maintains communication between inter, and intra-agency services providers for successful coordination of services;
- Maintains records of all activities;
- Attends required agency meetings and external meetings with other local providers;
- Assists in documenting program activities, and reports outcomes as required;
- Provides community outreach and education; and
- Performs all other duties, as assigned and necessary for operations.
Qualifications:
- BA degree preferred; AA and a combination of relevant experience may be considered.
- Bi-lingual English/Spanish required. Ability to read and interpret documents.
- Experience as a Patient Navigator, Case Manager, Education or Human Services and great customer service experience is preferred.
- Experience in HIV/AIDS services is preferred.
- Experience with the LGBTQ, African American, Latino and low-income populations.
- Specialized training or experience in HIV prevention interventions, PrEP/PEP, and STIs preferable.
- Experience working with youth and persons at high-risk for HIV infection.
- Experience working with substance abuse, socially, ethnically and economically diverse populations
- Experience in any educator role is preferred.
- Ability to work professionally and ethically within multi-cultural settings, including ability to maintain confidentiality and privacy of persons, documents, data, and communications.
- Sensitivity to HIV/AIDS and Gay, Lesbian, bisexual, and Transgender issues.
