We are currently seeking a dedicated and compassionate Case Manager to join our growing organization. The Case Manager is the first point of contact for all incoming clients and focuses on ensuring clients receive a warm introduction to Athena and ensures a smooth transition is made to their Clinician. The Case Manager works in collaboration with a multidisciplinary team of administrators, psychiatrists, and master's level clinicians. This individual may work out of our mental health clinic located at 2825 Third Avenue, Bronx, New York 10455 (Suite 402) or our 33 W 60th Street, New York, NY 10023 location.
Job Description
- Answer all incoming referral and potential client calls
- Determine client eligibility for Athena services via phone or in person
- Manage outreach needs for a caseload of incoming and current clients
- Collect client demographic information, previous mental health history, insurance information and details of requested services to determine appropriateness of fit for Athena
- Tracks and monitors client progress by overseeing weekly follow up until transition is successfully made to clinicians
- Verify Medicaid insurance eligibility through EPACES
- Schedule Intake appointments in EHR system
- Following up the morning of with clients to confirm intake appointments are successfully scheduled
- Check in with clients prior to the first therapeutic session to ensure they are confirmed and have all necessary equipment in order to connect with the Clinician and answer any questions that may arise
- Confirm clients on caseload have successfully attended their first
- Greet and schedule walk-in clients when needed
- Follow up with clients after intake appointments to ensure initial appointment is made with assigned Clinician
- Assist Case Manager Supervisor with incoming Clinician support when needed
- Collaborate with referral agencies and provide and update on referral status when needed
- Maintain data for incoming clients and collaborate with leadership staff, direct supervisor and other colleagues in a professional manner
- Maintain a high level of confidentiality in respect to all client information and internal documentations
- Utilize the agency's electronic health record system to ensure proper and timely completion of documentation and accurate client information
- Perform other duties as assigned by Supervisor
Qualifications
- Completed High School Diploma or Associates Degree or higher
- 1-3 years of relevant professional experience
- Bilingual in English and Spanish required
- Superior interpersonal skills and ability to work extremely well as part of a team.
- Ability to work well under pressure and manage sensitive or controversial clients with tact, kindness, and professionalism.
- Must be passionate about working with underserved communities and possess a high level of engagement, communication and interpersonal skills
Working Conditions
This position will be located at either our mental health clinic in the Bronx or administrative office in Manhattan. This is a professional work environment where a computer, desk, chair, and office supplies will be provided.
Physical Requirements
The physical demands described here are representative of those that must be met by an employee in order to successfully perform the essential functions of the role above.
While performing the duties of this job, the employee is regularly required to hear and speak on the phone or computer throughout the day, stand and sit for an extended period of time and be able to write and answer emails in addition to carrying objects no greater than 25 pounds, when needed.
