Position:  Case Manager 

Program: Fatherhood Initiative

Reports to: Program Director

Work Location: 1835 Sterling Place, Brooklyn, NY 11233

Hours: Full-Time - 35 hours per week Monday - Friday

Salary: $40,000 to $50,000

Serving more than 20,000 individuals annually, BCS is committed to a broad and diverse range of services including prevention of foster care placement; crisis intervention; early childhood and after school education; and vocational training, job placement, and clinical services to adults with physical and developmental disabilities, and histories of mental illness.

Program Summary:

The Fatherhood Initiative will provide young and older non-custodial fathers opportunities to interact through joint participation in parenting skills workshops, support groups, and special events with their children. Programs will also assist fathers, including those with prior involvement in the criminal justice system with achieving employment and educational goals and will integrate aspects of conflict management, violence prevention and other support services to support fathers with resolving issues and obtaining economic security.

Responsibilities:

Responsible for participant recruitment and retention

Facilitate group workshops

Conduct outreach (with CBO’s and throughout the community) to identify appropriate Fatherhood Initiative participants.

Meet with potential enrollees individually and in groups

Track attendance/participation levels and following up with participants begin to disengage to encourage their continued participation.

Provide at least 3 months follow-up support after participants complete or exit the program. This includes meeting with program graduates and their families, employers or other support network to assist them in achieving their personal goals and strengthening family engagement; identifying/addressing challenges for graduates during follow-up period; working with other providers to establish/maintain linkages; assisting

Fatherhood Initiative case management team members in facilitating job/internship placements and service linkages;

Document and report on follow-up services and the results

Other duties as assigned.

Qualifications

BA/BS degree, AA degree and/or certified trained peer credential required, with experience (including lived experience providing support services to youth/adults)

Experience working with and delivering case management services to vulnerable populations.

Ability to build and maintain trusting relationships

Ability to maintain confidentiality

Sensitivity to the needs of participants and program graduates;

Experience recruiting and engaging with fathers

Experience in Maternal and Child Health issues is a plus.

Experience working with diverse populations, communities, and organizations.

Strong interpersonal and organizational skills

Excellent written and verbal communication skills
