Position Description

The Licensed Practical Nurse provides nursing care to patients in the home setting. This care encompasses a wide range of med/surg nursing procedures and involves monitoring patient status and compliance, performance of procedures and teaching of patients and caregivers in preparation for discharge. All care delivered is based on Standards of Practice for the particular state of practice and must be in compliance with company policy and regulatory requirements.

Primary Responsibilities:

  • Maintains current clinical knowledge and expertise.
  • Schedules in-person wellness visits.
  • Completes consumer consents and initial service questionnaire in person.
  • Completes wellness visits every 90 days for each consumer on their assigned caseload.
  • Documents all service questionnaires, wellness visits and incident reports in accordance with the Care Coordination policies.
  • Building initial service plans, reviewing service plans every 90 days or as needed and making addendums to service plan as needed
  • Delivers quality nursing care through appropriate interventions and observations.
  • Ensures that all new employees are given the QUANTIFERON TB Testing upon hire with appropriate follow-up per federal, state & organizational policy.
  • Safely administers and documents medications as prescribed & in accordance with federal, state & organizational policy.
  • Secures and maintains employee records following federal (including HIPPA) state & organizational policies.
  • Performs procedures according to company policy and PA Nurse Practice Act.
  • Determines issues requiring escalation to Director, Medical Director and/or BLessings4Ever Home Care Agency as appropriate.
  • Maintains established productivity standards on a regular basis.
  • Follows organizational policies and procedures regarding infection control, disposal of hazardous waste and use of protective devices as evidenced by documentation and the absence of related incident reports.
  • Ensures that medications are stored appropriately & temperatures are documented according to department protocols & policy.
  • Completes annual & assigned trainings in a timely manner.
  • Administer medications and treatments as prescribed.
  • Adheres to professional standards & follows policies and procedures of Blessings4Ever Home Care Agency.
  • Abides by federal and state requirements, HIPPA guidelines, and accreditation standards.
  • Performs related duties as required.

Education/Experience Requirements

  • Graduation from an accredited school of vocational/practical nursing. Must possess a valid/active practical nursing license in the state(s) of practice.
  • A minimum of two-year experience in medical/surgical nursing is required. Previous experience of 1-year in a long-term care facility is also required.
  • Must have a valid driver license and vehicle registration.
  • CPR Certification

Physical Demands & Environment

Employee may be required to sit, bend, stoop, use keyboard, see, talk, and hear. May occasionally lift objects of 10 to 25 lbs. The position is primarily based in the home of the patient.
