Job Purpose: The care coordinator is responsible for providing administrative support to clinical staff across the multitude of programs and initiatives at our community health center. In addition, they serve as a community liaison between the health center and the patient. This individual will serve as the primary contact between providers, reports, and Director of Programs; with the aim to assist, monitor, and identify health care gaps.
- Collaborate with co-workers and supervisors to increase productivity and develop an action plan for executing responsibilities and duties involved in programs, initiatives, and projects.
- Attend meetings as applicable to the job title. (i.e. all staff meetings, wellness center meetings)
- Be well versed on the services and programs offered at Families Together, including Family PACT, Every Women Counts, CHDP, Tile X etc…
- Attend all health fairs, company events (Farmer’s Markets) and other activities as assigned.
- Develop relationships with outside organizations and individuals for them to act as referral sources of patients.
- Create, print, and distribute company collateral (marketing materials) specific to programs.
- Ensure all appointments are properly created, scheduled, and confirmed.
- Assist in providing transportation and social services to all patients.
- Monitor and review an assigned panel of patient profiles and authorizations; as a means to address their healthcare needs and meet clinic measure goals.
- Prepare monthly productivity reports and track goals with direct supervisor.

Educational/Credential Requirements:
- High School Diploma or Higher
- BLS Certification
- Associate Degree
Position Qualifications, Skills, and Experience:
- At least 2 years working for a community-based organization or community health center
- Previous experience in the medical field
- Familiarity with electronic medical records
- Knowledge and experience in working with diverse communities in the Orange County.
- Familiarity with marketing tools such as flyer creation and other marketing tactics
- Skilled in meeting project deadlines. Great time management skills
- Exceptional organizational and planning skills. (Must have 1 year minimum of experience organizing events)
- Reporting & Data Analysis (Excel)
- Bilingual speaking preferred: Spanish
- Strong writing and oral presentation skills.
- Experience in facilitating public meetings and other public forums
- Exceptional interpersonal skills
- Active Listening, Social Perceptiveness
- Available to work occasional evenings and some weekends as needed to attend community events and meetings.
