H&W: $4.41/ hour This position is eligible to receive a Health & Welfare Benefit Rate per hour on all hours paid up to 40 hours per week and not to exceed 2,080 hours per year.

Job Summary:
This position provides case management of the Young Adult Case Management Program (YACMP) participants from entry into the program to release by administering the program’s policies and procedures in compliance with contractual obligations.
