JOB SUMMARY
The Community Health Worker (CHW) will work with Community Based Organizations (CBOs), healthcare providers, and departments/programs within the County to strengthen health equity, serve as a liaison between community members and service providers, address social determinants of health, and provide education in culturally relevant and linguistically appropriate ways.
MINIMUM QUALIFICATIONS
Bachelor's Degree from an accredited college or university in Community or Public Health, Education, Health Education, Public or Health Administration, or closely related field OR an equivalent combination of related education and experience working as a Community Health Worker, Promotion, Health Navigator, Patient Advocate, or similar. Community Health Worker Certification strongly preferred.
Must have a valid Utah driver's license, auto insurance, and reliable transportation
It is mandatory that all new hires receive the TDaP vaccine before beginning employment or provide a copy of their immunization record prior to starting employment. The immunization record must show the name of the vaccine and date received.
Upon hire, a two-step Tuberculosis skin test will be required. The first will need to be place prior to starting employment.
Experience working in specific zip codes/neighborhoods and with underrepresented communities strongly preferred.
Ability to read, write, and speak in Spanish strongly preferred
