The Patient Care Coordinator will be responsible for ensuring patients receive the necessary care, guidance, and direction based on medical guidelines and operational protocols. They will also be responsible for ensuring patients comply with necessary treatment, coordinate with various departments to address patient needs/concerns and facilitate all aspects of care between patients and physicians.

Essential Functions:

  • Reach out to and follow-up with patients to ensure they comply with our medical recommendations and receive the proper attention and care to achieve the best possible health outcomes.
  • Coordinate as needed with various departments to address patient concerns as needed to facilitate the proper care and coordination to the patients in a timely manner.
  • Ensure that all necessary and relevant patient information is being collected and recorded accurately in the patient’s chart.
  • Receive ongoing training on the EMR System, Clinical and Operational protocols, and proper patient communication and satisfaction methods
  • Inform, instruct, and educate patients about their healthcare needs to positively impact their health and overall well-being.
  • Proactively oversee the Providers’ daily schedule to take immediate action as needed to reach daily Clinical and Productivity goals.
  • Maintains and ensures the confidentiality of all Personal Health Information (PHI) collected and disseminated, in accordance with HIPAA requirements

Education and Requirements:

  • Bilingual (fluent in English and Spanish) (Required)
  • High School Diploma or Equivalent (Required)
  • Call Center Experience (Preferred)
  • Healthcare Experience (Preferred)

Job Type: Full-time

Pay: $31,000.00 - $42,000.00 per year
