Social Worker, MSW (Spanish)

Golden Years ADHC is looking for a Bilingual Social Worker/Social Worker Assistant who is responsible for daily interventions and care planning of clients with multiple medical and psycho-social problems. The Medical Social Worker/Social Worker Assistant plans for and monitors services, interventions ensuring provision of quality care, plays role of a liaison between participant, family, caregivers and professionals.

MINIMUM QUALIFICATIONS

• Master’s degree or Bachelor’s degree from accredited school in Social work, Psychology or human related services
• Bilingual English / Mandarin or Spanish
• Ability to work with individuals with physical or mental limitations

ESSENTIAL FUNCTIONS/RESPONSIBILITIES

• Timely documentation of assessments, reassessments, quarterlies, flow sheets for each participant.
• Researches and coordinates community referrals.
• Monitors and reports case recording on a timely basis.
• Participates/leads in coordination of social/health services.
• Participates in Multi Disciplinary team meetings and coordinate care of each participant.
